It is a legal document that communicates your health care decisions and wishes to your attending physician and medical personnel when you are physically or mentally incapacitated to do it yourself. This document also includes Medical Power of Attorney, that lets you assign an agent, a trusted adult you authorize to receive all the pertinent information from anyone involved in your medical care and make the medical decisions on your behalf, once you become incapacitated to do it yourself.
